Permanent Supportive Housing

Our Permanent Supportive Housing program is for chronically homeless individuals and located in various locations throughout Houston.  We provide onsite substance abuse case management, intensive life-skills, and mental health counseling. Program participants are provided with resources that focus on enhancing their abilities to exit homelessness and to remain self-sufficient. Clients may remain in the program as long as necessary with no maximum time limit.  The goal is to provide support and skills that help sustain stable employment/income, housing, and life skills. Permanent supportive housing is a HUD funded program operated by Harmony House, Inc.

Participants have access to:

  • Substance Abuse counseling and support groups
  • Mental Health counseling and support groups
  • Life Skills

Eligibility:

Access to this program is coordinated through The Way Home’s Coordinated Entry (CE) system, which is the regional process used to help people experiencing homelessness connect to available housing and support services. People begin the Coordinated Entry process by connecting with a homeless service provider or outreach team within The Way Home network. If appropriate, a standardized assessment may be completed to help determine next steps. Housing and services are not guaranteed, and eligibility depends on individual circumstances, program availability, and location. Demonstrating an unwavering commitment to maintaining her sobriety, Teresa recently celebrated two huge milestones on the same day. “I have been two years housed and two years alcohol free. It’s a very special day for me.” she said.  An avid gardener, Teresa spends much of her time tending to her beautiful patio garden, along with her two “kids” – a fish named Gizmo and a dog named Violet.
